Details on Underwriting & Confidential Information Information concerning your insurability will be treated as confidential. When considering whether you and the persons named in your request for insurance qualify for coverage, we will rely only on the information you furnish and on any information we may ask you to obtain from your doctor, other practitioners, hospital, other facilities, other insurance companies, etc. New York Life will not disclose such information to anyone unless you authorize it, or where required or permitted by law. This information may be seen by New York Life and Group Policyholder (USBA) employees, but only on a need-to-know basis in considering your request for coverage. Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved. All coverage is subject to underwriting approval by New York Life. Effective date of insurance: All coverage is subject to underwriting approved by New York Life. Approved coverage will take effect on the approval date specified by New York Life, provided the premium has been paid. You and your spouse must also be performing your normal activities on the date of approval. Dependent children must not be hospital confined on the date of approval. Payment of premium does not mean coverage is in force before the effective date specified by New York Life. If we can't provide the coverage you request, we will tell you why. If you feel our information is inaccurate, you will be given a chance to correct it or complete the information in our files. Although we cannot generally provide you with any medical information in our files, we will, upon written request, provide such information to a physician you designate. Other Plan Provisions Continuation of Coverage: Your Joint Level Term Coverage may be continued so long as both you and your spouse are under age 70, you pay your premium when due, your USBA membership is continued, and the Group Policy is not terminated or modified to end your coverage. Earlier termination will occur upon divorce or annulment of your marriage, the death of you or your spouse, or if you or your spouse formally requests voluntary termination. Coverage for your children continues as long as yours does, so long as they remain eligible. If you should die, your insured children may continue their coverage provided they remain otherwise eligible. Individual Certificate: This website outlines the principal provisions of the USBA Joint Level Term Family Group Life Insurance Plan. Complete details are in the Group Policy G-5393-1/-O (Policy Form GMR) delivered to USBA at its headquarters in Kansas City, by New York Life. Each member receives a certificate which summarizes policy provisions affecting his or her insurance.
