Note: This plan is not currently available in New York.

You can complete your application online or print an application and complete it at your convenience. Plus, when you
choose to apply online, we’ll ask you to decide if you want to:
- Submit Electronically
- Print and Mail
(After completing the application, you can print
it, sign it and mail it to us with a check for
the required premium amount. If we cannot issue
a policy for any reason, we will refund the amount
you paid in full.) |

Current Monthly Premium Rates
PLEASE NOTE: Coverage is available to Members/Associate Members from a minimum of $25,000 to a maximum of $500,000 in $1,000 increments. (Note: $100,000 minimum for Preferred Rates.)
These are current policy premiums based on the member’s/associate member’s age, sex and smoking status. Although not guaranteed, premiums for the first 10-year term are expected to remain level.
Renewal rates for subsequent 10-year terms (available upon request), will be based on your then attained age, and coverage will be renewed regardless of your health.

Montana Residents: Male rates apply to all Montana residents regardless of sex.
