- Days 91+ – 100% (maximum monthly benefit $3,750)
- Radiation and Chemotherapy – $750 (maximum benefit per illness period)
- Ambulance – $37.50 per one-way trip
- Transportation – $200 (maximum benefit per round trip)
- Extended Care Facility – $50/day
Preferred
